This is AFTER you are placed on a team 

depends what level you will be playing.  We will go over this at 

the parent meeting.  October 24th.

1.  AAU membership (VTA needs a copy)

2.  AAU medical release form

3.  Copy of Birth certificate

4.  VTA Player/Parent agreement
